In an ongoing effort to serve our patients more effectively and efficiently, we have established the following office policies and procedures. To assist us in our ability to provide you with excellent care, please read the following information carefully. If you require clarification or have any questions, please consult with our receptionist.
You may contact the receptionist at 425-219-4325 during clinic hours. When the clinic is closed, you may leave a message with our answering service.
If you are an established patient and have a health concern that needs an immediate response and you need to contact the doctors, after hours, you may call the office and speak with our answering service, they will page your physician directly. There is a $65.00 charge for this service. In a medical emergency, go to an emergency room or call 911.
Scheduled appointments may be cancelled up to 24 hours prior to the appointment time. Thereafter, $125.00 will be charged for the scheduled appointment and for “no-shows”
We do not accept walk-ins. All appointments must be made in advance by phone or after a visit. Routine visits are scheduled within 1-3 weeks. First visits are scheduled within 2 weeks. Emergencies are scheduled within 2 days depending on severity.
First office visits are usually about one hour long. Return visits are usually about 30 minutes long. We recommend scheduling your visits ahead of time, as the schedule fills quickly.
Pharmacy Return Policy
Pharmacy items that have been opened cannot be returned. Pharmacy items that are returned unopened and with the seal intact are subject to a 18% restock fee. Before an item is returned Lakeview Family Wellness staff will discuss the reasons for the return and will then correspond with you further. Any items that are accepted as a return will be put on the patient’s account as a credit. Lakeview Family Wellness will not refund cash, check or charge back to credit card.
We ask that you give our office staff two weeks advanced notice on pharmacy items you will need.
Patients without health insurance will be charged a sliding-scale rate on their visits as well as pharmacy items. Payment in full for all clinic services is expected on the day of service. Patients with health insurance will be charged out of pocket for all pharmacy items, copay or patient responsibility from the insurance. We do not carry balances.
We take care of all insurance billing.
Forms of payment: Payment may be made by cash, check or major credit cards.
Alternare, Uniform Medical Plan, United Healthcare, Cigna PPO and Cigna Great West, Aetna U.S. Healthcare, Premera Plans, KPS, First Choice, American Whole Health Network, Group Health. You are responsible to verify your own benefits, obtain a referral if necessary and pay for all services not covered. We cannot guarantee your benefits until we have processed your visit claims. We do not accept primary/secondary insurance, if that is what you have, you will be responsible to pay up front for the services you receive at The Health and Wellness Institute or bill the secondary insurance yourself, we will give you a fee slip that you can send to your insurance company. This does not guarantee reimbursement by your insurance company. Medicare does not cover any of our services.
If we are not specifically contracted with your insurance company, you may ask them if you have benefits for out-of-network naturopathic care. Also, remember that some insurance companies may put limitations on how often you can receive naturopathic or osteopathic care.
*If you have an HSA account or flexible spending plan, be sure to check what benefits you have available.
First Visit Information
When coming in for your first visit, you will need to bring in copies of your medical records, as well as lab results performed within the last 6 months. Please plan to arrive 15 minutes before your scheduled visit in order to fill out paper work, which will help the physicians be as timely and efficient in serving you as possible.
